It is a computer form... and the instructions say to PRINT legibly.. like you can take your pen and write on the computer! ROFL!!!

I absolutely can't figure it out but maybe if I TYPE legibly YOU can figure it out. Good grief! Someone didn't think before making that form, huh? LOL! LOVE YOU!
